Every child, youth, and their family is unique. We can help by recommending the type of service that best suits your unique needs. You may be connected with an Intake and Referral Consultant, an FASD Consultant, or a Coordinated Service Planner on our team that can help you navigate services.
Access Service
Areas served: Hamilton
Ages served: birth to age 18
Our Access Service provides information, consultation, intake, and referral to Ministry of Children, Community and Social Service (MCCSS) funded services for children and youth that are presenting with developmental, FASD / alcohol related neurological disorders, Autism, and / or complex needs.
Our Access Service can also provide information about transitioning to adult developmental services, if you are aged 14 years and older.

Coordinated Service Planning

Ages served: birth to age 18 or school exit
Our Coordinated Service Planning program assists children and youth with multiple, complex needs, and their families, to coordinate their multiple services, and develop one plan from which the family and involved service providers will work.

FASD Consultation Service
Areas served: Hamilton
Ages served: birth to age 18 or school exit
Our FASD Consultation Service provides youth and families with: information about FASD and alcohol related neurological presentations including evidence-informed strategies; linking to available services; coordinating services; and linking families to caregiver supports.
Our FASD Consultation Service is a member of the Hamilton FASD Collaborative https://www.fasdhamilton.ca/. The Hamilton FASD Collaborative provides the following services: child/youth-specific case conferences; in-service education for professional; and training and education. Our FASD Consultation Service can link families to these services.

Consent
In order for Contact Hamilton to provide our services, we require consent from the appropriate person.
The youth, or someone on their behalf and with their consent, may refer the youth to Contact Hamilton.
We accept verbal and written consent.
- Youth aged 12 years of age or older can request counselling services without the consent of their guardian if they wish
- If you are referring a youth 16 years of age or older, consent from the youth is required, where the youth has the capacity to provide consent
If you are referring a child or youth under the age of 16, consent from the youth’s legal guardian / custodial parent is required.
Custody
We will need to verify custody information in order to confirm from which parent(s) we are required to obtain consent. Our Contact Hamilton team member will walk you through this process if it applies to you.

If the child / youth is experiencing mental health related needs, please contact Lynwood Charlton Centre’s Access and System Navigation (ASN) program: https://lynwoodcharlton.ca/. The ASN program provides access to Ministry of Health funded mental health services for children and youth.
